Access Service Request (ASR) Requirement for Interconnecting Carriers
As set forth by the Alliance for Telecommunications Industry Solutions (ATIS) and TELCORDIA standards, the Access Service Request (ASR) is a form used by a Competitive Local Exchange Carrier (CLEC) to request that the Incumbent Local Exchange Carrier (ILEC) provides Special Access or Switched Access Services as specified in the various Access Service Tariffs. Effective immediately, Windstream will require interconnecting companies to complete an ASR to submit to the Windstream Interexchange Carrier Service Center (ICSC).

Mergers and Acquisitions Process
The Mergers and Acquisitions Process is designed to assist Wholesale Customers during their merger, acquisition and consolidation of accounts.
The process begins when the acquiring Competitive Local Exchange Carrier (CLEC) contacts Windstream Wholesale Services to determine how the transaction will be managed. The acquiring CLEC will also be responsible for providing a spreadsheet inventory of the transferring assets. The CLEC will be provided a checklist to guide them through the process.

Local Service Providers are now invited to form a relationship with Windstream.
Industry Requirements
Before entering into a business relationship with Windstream, a Local Service Provider must comply with a series of industry, regulatory and legal requirements. These may include, but are not limited to:
- Obtaining a Certificate of Public Convenience and Necessity (CPCN) or other licensing certificate from state commission, where required. Contact your state commission regarding certification requirements in the state where you plan to conduct business.
- Secure appropriate industry codes (e.g. OCN), as required.
- Secure an Exchange Carrier Code from Telcordia Technologies, Inc.
- Obtain copies of the Articles of Incorporation and Name Registration from the Secretary of State in the state which you are incorporated.

Steps to Executing the Agreement
- Download and submit a completed request form to become a Local Service Provider. Please indicate the state and type of agreement you would like to execute. Please have all documentation listed above available for submission with your request. You can fax or mail your request to:

- Once you receive an executed agreement, please file your request with your respective state commission for approval.
- When submitting your Competitive Local Exchange Carrier (CLEC) Profile to the LSPAC Center, you will need to have the following information available:
- Financial information for credit verification
- Tax certificates (if required)
- Pay deposits (if required)

- Review list of approved ILEC Interconnection Agreements for Texas, if applicable.
